A 1/ A 2, Phoenix, AZ 85021 (Additional Language Differential Pay Available) Full Benefits Package, including 401 K Generous PTO/ Sick Time (4 weeks in year 1) Pay starting at $17.18/hour DOE Responsibilities: The Health Navigator will work within our Federally Qualified Health Centers (FQH - Cs) to support patients in accessing and navigating healthcare services with a focus on the Medication to Address Opioid Use Disorders (MOUD) program and closing essential gaps in care. This role is essential in improving health outcomes by providing education, resources, and guidance for underserved populations, including adults with Serious Mental Illness, uninsured, underinsured, and vulnerable community members. The Health Navigator collaborates with healthcare providers, case managers, and external community resources to promote patient-centered care and facilitate access to essential health services. The Health Navigator will provide BHT level behavioral health and skills interventions to facilitate overall wellness and health to patients served. This position reports to the Health Center Director or Clinical Site Manager. Associate’s degree or Bachelor's degree in health education, social work, or a related field preferred. A minimum of 1.5 years behavioral health experience. Experience working with underserved populations is strongly preferred. Peer Support Employment Training certification preferred. Bilingual skills (e.g., Spanish, Mandarin, or other languages) are highly desirable but not required. Must have valid Arizona driver’s license, be 21 years of age with minimum 3 years driving experience, and meet requirements of Terros Health’s driving policy Must pass a TB Test.
