This position ensures continuity of sex offense specific treatment and monitoring for sex offenders transitioning to parole or community corrections on inmate status. This position will work with the facility parole and community parole officers to provide relevant information related to DOC sex offender treatment. Job duties include, but are not limited to:Work with the DOC to ensure the community parole officer and the community treatment provider receive the transitioning offender’s SOTMP Discharge Summary & recommended sex offender specific parole conditions. Collect and verify all offense specific treatment collateral is complete and provide any clarification needed to community parole and the receiving therapist, ensuring continuity of sec offender treatment and monitoring. Ensure continuity of care by; following SOMB Standards and Guidelines, SOTMP therapist manuals and protocols, writing reports and providing consultation to staff and receiving agencies; conducting workshops and follow-up contact with the family and community support team of the sex offenders. Create and maintain the the SOTMP Continuity of Care Discharge File for each SOTMP (Sex Offender Treatment & Monitoring Program) enrolled client and ensure recommendations and risk levels are communicated to community parole officers and other members fo the Community Supervision Tean (CST). Build and maintain SOTMP continuity of care databases which support offender placements and recommendations for SOTMP. Review and suggest revisions to the discharge summary based on updates to SOMB Standards and Guidelines, or changes made within the SOTMP to ensure compliance. Review clinical notes, evaluate treatment progress (meeting with the offender as needed) and interpret risk assessments to determine level of risk in order to collaborate and share information and recommendations with community stakeholders to secure appropriate community resources. Participate in Sex Offender Treatment & Monitoring Program leadership meetings to design & update program content, receive training, conduct staffings on offenders throughout DOC to determine treatment recommendations for SOTMP and other behavioral health needs in order to make appropriate referrals. Provide the SOTMP treatment team & DOC staff, community stakeholders with training and education on continuity of care and community resources for the sex offender population. Act as an SOTMP representative as needed in stakeholder meeting and interviews of offenders as a program expert who can provide participation summaries and continued treatment recommendations. - - YOU ARE ENCOURAGED TO PRINT A COPY OF THIS ANNOUNCEMENT FOR YOUR RECORDS - -
