What you will Do: Supports the team in identifying patient needs and implementing the outcome/goal-based plan of care. Assists the patient care team in the delivery of care. Hours/ Schedule: .6 FTE, two 12 hour shifts per week. Every third weekend rotation 6:45 am to 7:15 pm. Minimum Qualifications: Verification (by transcript or certification) of successful completion of Nurse Aide I course; successful completion of Nursing Fundamentals course or successful completion of the Competency Test. Nurse Aide II-Advanced course is preferred and may be provided post-hire. In Home Health must have completed Nurse Aide I course. Requires the ability to read, write and understand the English language, and communicate effectively with patients, visitors, staff and physicians while performing their job duties. Basic language, basic math, basic to intermediate reasoning ability. Basic computer skills required. Ability to work with diverse groups of people.
