The Nurse Practitioner will collaborate with a dedicated clinical care team to deliver evidence-based primary and family care to a defined panel of patients. The care team partners daily with providers to drive improved health outcomes through comprehensive assessment, diagnosis, treatment, chronic disease management, health counseling, and disposition planning. Schedule: - 32.5 hours per week (average) - Includes every other weekend - Possible holiday rotations Pay: $74.87/hr paid weekly Key Responsibilities - Provide evidence-based primary and family care, including: o Accurate assessment, diagnosis, and treatment o Management of acute and chronic conditions o Health counseling and patient education o Disposition planning - Perform routine examinations and diagnostic testing - Coordinate with healthcare professionals to ensure comprehensive care plans - Encourage patients to actively participate in their healthcare through screenings, wellness services, and chronic condition management - Conduct administrative duties including: o Opening and closing the clinic o Inventory management o Insurance verification and payment collection o Patient follow-up o Maintaining a clean and safe clinical environment Required Qualifications - Master’s Degree from an accredited Family Nurse Practitioner program - Current National Board Certification - Active, unrestricted Advanced Practice Nurse license in the state of employment - Active and unrestricted NP/ PA license in good standing - Current BLS certification from ARC or AHA (must provide copy upon offer acceptance) - DEA registration application required upon offer acceptance (unless otherwise noted) - Verifiable High School Diploma or GED Preferred Qualifications - Minimum of 1 year recent primary care experience as a Nurse Practitioner in family medicine or primary care practice o If no primary care experience, successful completion of primary care training program required - Working knowledge of quality measures in chronic disease management - Experience with Electronic Medical Systems (EMS) - Ability to practice autonomously without direct supervision - Strong organizational and multi-tasking skills - Excellent verbal, written, and electronic communication skills - Self-motivated with strong problem-solving ability - Comfortable working remotely within a clinical care team culture - Proficient in information management and technology - Willingness to obtain multi-state licensure - Ability to collaborate frequently with professional colleagues to ensure quality patient care JOB ID 168840
