Public Health – Seattle & King County - Community Health Services Division is seeking a Project/ Program Manager III (PPM III) (Referral Coordinator) to provide support in our Parent Child Health programs. The Referral Coordinator will support and advance the Community Health Services Division's values of delivering essential, high-quality, culturally relevant services to our King County communities. The successful candidate for this position will demonstrate experience, knowledge, and skills in project management, quality improvement, and communications. They will ensure the work is aligned with CHS's priorities, with particular focus on pregnant and parenting individuals, children, and families. This position will be the main point of contact for the internal connections between programs within our King County Public Health system and our Parent Child Health programs. They will use their experience and skills in interagency, inter-jurisdictional, and multidisciplinary coordination as well as planning and project management to lead the work of increasing access to our Parent Child Health services for the clients who are already connected to one or more of our Public Health programs, such as preventive and primary care services, oral health services, homeless healthcare, and youth health services. This position will also be a contact for MAC funding, NFP grant monitoring. This position will also serve as a liaison First Steps with our staff working at public health clinic sites and be a subject matter expert for the First Steps program policies and procedures. To learn more, please visit Work Schedule. Typical work schedule is Monday through Friday, 8:00 am- 5:00 pm, with the occasional weekend depending on business need. This is an exempt position and is not subject to the Fair Labor Standards Act and is not overtime eligible. Remote and Onsite Work. The PCH Admin team works in a hybrid model, with days in the office as well as telecommuting. The ratio of remote to onsite work will be dependent on business needs and is subject to change. The primary onsite location is Chinook Building, located at 401 5th Ave, Seattle, WA 98104. Employees must reside in Washington state and within a reasonable distance to their King County worksite to respond to workplace reporting requirements. King County has a robust collection of tools and resources to support working remotely. Employees will be provided with a County-issued laptop and must maintain a home workspace with an internet connection where they can reliably perform work and remain available and responsive during scheduled work hours.
Job Duties
Project Management. Works independently on projects assigned by supervisor, develops new projects based on gaps in system. Inter-jurisdictional collaboration with public health teams and community partners at various sites throughout King County to review workflow and processes related to connections between programs. Develop and manage PCH internal referral system in conjunction with Epic Practice Management System tools. Design and implement standardized workflows to ensure seamless patient transitions between internal departments. Data Reporting and Analysis. Performs moderately complex reporting and analysis of data related to projects and program. Communicates to various stakeholders within our multidisciplinary team and our community partners as needed about data related to programs, workflow, and processes. Program Liaison to Public Health Centers. Communicates with stakeholders including community partners, and Public Health professionals about system processes, applications and modifications as needed. Ongoing interagency work build and maintain professional relationships with staff at public health centers, community partners, and other programs in PHSKC. Working to identify gaps and create solutions for all those eligible to better access PCH services. Technical Expertise. Serving as an expert technical resource for internal connections between programs in our Parent Child Health programs. Data entry, monitoring, reporting, and on-going testing with database software such as Tableau and Excel and automated systems such as Power BI. Other duties as assigned. Able to shift work to meet emerging priorities.
Experience, Qualifications, Knowledge, Skills

The most competitive applicants will have the following experiences and qualifications (Desired):Recent experience, of one year or more, working in the First Steps, WIC, Nurse Family Partnership or other maternal and child health programs. Advanced skills using database software such as MS Excel and automated systems such as Power BI. Advanced skills in driving forward projects with multiple stakeholders including direct service staff, clinic supervisors, community partners and others.
