Specialist – Behavioral Health Support (BS)Shift: Full-Time, Days. Location: Primary Work Location (Frankfort primary care on M-W; Ferry Peds in Lafayette on Th/ F)Position Overview:Provides behavioral health support for patients in collaboration with primary care and licensed behavioral health clinicians. Facilitates patient involvement in treatment and education, serving as a liaison among medical staff, patients, families, and outside agencies. Performs mental health triage and evaluates psychosocial risk factors under clinical supervision. Implements evidence-based individual, group, and family interventions, and assists with discharge planning, utilization review, and case triage. Acts as a point person for abuse, neglect, and domestic violence cases, coordinating with protective teams and community resources. Key Responsibilities:Collaborate with healthcare team to ensure safe, timely, and resource-appropriate patient care. Conduct mental health triage upon provider referral, evaluating psychosocial risks. Perform evidence-based behavioral health interventions for individuals, groups, and families. Assist with discharge planning and utilization review under supervision. Serve as liaison for cases involving child and adult abuse, neglect, and domestic violence; coordinate with protective agencies. Develop strong relationships with community agencies, law enforcement, schools, and legal entities. Maintain thorough documentation in electronic medical records. Qualifications & Requirements:Bachelor's Degree in Social Work, Psychology, Human Services, or related mental health field .-3 years of relevant healthcare, mental health, or community experience. License aligned with IN state code IC 25-23.6-4-1; LBSW required for BSW holders. Certification in crisis intervention may be required; BLS certification may also be required. Knowledge of therapeutic interventions, crisis de-escalation, and legal mandates for protective services. Understanding of growth and development principles across the lifespan. Ability to provide a safe, therapeutic environment and facilitate daily activities. Basic proficiency in Electronic Medical Records and MS Office (Word, Excel, PowerPoint). Strong relationship-building skills with community, healthcare, law enforcement, and social agencies.
