Bachelors Degree required or Masters Degree Social Work is required? ? 3 years staff nurse at an acute care hospital (if RN) required or? ? 3 years hospital/medical social work (if Social Worker) required or? ? 2 years hospital discharge planning/care management required and? ? Case Management supervisory background preferred? ? RN Registered Nurse upon hire required? ? LMSW - Licensed Master Social Worker is required upon hire? ? CPR - Cardiopulmonary Resuscitation (BLS) is required upon hire? ? ACM - Accredited Case Manager OR CCM - Certified Case Manager is preferred upon hire or other ANCC preferred upon hire. In addition to the qualifications, a successful Care Transition Supervisor will model the factors listed below:Working knowledge of medical necessity criteria preferred. Knowledge of Microsoft Outlook and Office (Word, Excel)Customer service skills. Ability to engage in complex clinical decision-making. Strong oral and written communication skills.
