The Licensed Behavioral Health Professional is responsible for providing assessments for assigned adolescent, adult and geriatric patients. This job is also responsible for telephone and in-person crisis intervention and counseling for patients and family members and for information and referral services in the Psychiatric Emergency Center. This job participates in the development, implementation and evaluation of the treatment plan, acting as a liaison for family members, community agencies and the Mental Health Court, and the coordination of discharge planning activities and referrals. Essential Job Functions & Accountabilities: Gathers pertinent information about all assigned patients. Completes an assessment, including an assessment of discharge planning needs and alternatives for each assigned patient, per policy. Coordinates incoming and outgoing transfers and demonstrates knowledge of EMTALA laws when processing patient transfers. Participates in treatment planning for assigned patients. Coordinate filings for court-ordered mental health services with patients’ families and the Trinity Springs Pavilion Legal Coordinator. Charts assigned patients per operating procedure and maintains other records as assigned. Liaisons with patient’s family or other support systems regarding the patient’s care, treatment plan, and discharge plans. Assumes responsibility for crisis counseling, psycho-educational services, or other responsibilities as assigned. Assumes primary responsibility for discharge planning, including assessment of needs, coordinating referrals, consulting with other service providers, and consultation with other community resources, as appropriate. Maintains current knowledge about issues affecting the adolescent, adult and geriatric population. Maintains current community resource information regarding services for adolescent, adult and geriatric patients. Maintains clinical competence for adolescent, adult and geriatric Social Work services and exhibits professional growth and development. Ensures compliance with patient HIV pre-counseling and post-counseling per policy. Job description is not an all-inclusive list of duties and may be subject to change with or without notice. Staff are expected to perform other duties as assigned. Qualifications:Required Qualifications: Master’s Degree in Social Work or Counseling from an accredited college or university. Must have a current licensure in one of the following: Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) from the Texas State Board of Social Work Examiners or the Texas State Board of Examiners of Professional Counselors. Certification in non-aggressive crisis management; must be obtained while in orientation and maintained throughout employment. Current JPS recognized Cardiopulmonary Resuscitation (CPR) Certification; must be obtained within 90 days from date of hire. Preferred Qualifications: Licensed Clinical Social Worker (LCSW).
