This Part-Time OPS position will support the DOH-Flagler Office of Community Health programs for an estimated 20 hours a week, through an integrated and strategic approach to community outreach and projects directed toward prevention, assessment, risk reduction, and health status improvements for individuals, families, and the community. The position will utilize appropriate methods for interacting effectively and professionally with persons of all ages and from diverse cultural, socioeconomic, educational, racial, and ethnic backgrounds, lifestyles, and physical abilities. The part-time position responsibilities include acting as a support facilitator and delivering evidence-based Diabetes Prevention Program curriculum to multiple group cohorts, while promoting program participation through targeted outreach and engagement strategies. This position will support a wide range of outreach activities designed to promote public health awareness and improve access to care, working closely with community partners to implement health education initiatives and DOH-Flagler services. Responsibilities include participating in community outreach events, partner meetings, and health fairs to advance program goals, as well as assisting in the planning and coordination of health-related staff trainings. This part-time position participates in general staff meetings, staff development activities, and continuing education to support ongoing professional growth and departmental effectiveness. The position also includes performing related duties as needed, including assisting with public health services during disaster response efforts. Additionally, the position ensures the timely submission of EARS and timesheets to maintain compliance with administrative requirements. Other duties may be assigned as needed.
