We have an exciting opportunity to join our team as a PGY 1 Pharmacy Resident - Brooklyn. In this role, the successful candidate The New York University Langone Health (NYULH) Pharmacy Residency (PGY 1) is a 12-month program that will provide the resident with an intensive and individualized experience in the in-patient setting. The goal of this residency is to enable the resident to develop the skills necessary to provide a high level of pharmaceutical care in a variety of practice settings. The resident will be an integral member of the medical team, collaborate on patient care, participate in medical emergencies, and provide pharmacotherapeutic and pharmacokinetic consultation. After completing this residency, the pharmacist will have gained the knowledge and experience to be a competent general pharmacy practitioner. This program follows the guidelines and accreditation standards of the American Society of Health-System Pharmacy (ASHP). Job Responsibilities:To fulfill the core values and mission of NYU Langone Health and incorporate them into the performance of duties. Develop and maintain a high-level clinical pharmacy practice. Participate in multidisciplinary, bedside rounds. Pharmacotherapeutic and pharmacokinetic and pharmaco-economics consultation. Medical response team, airway, stroke and acute coronary syndrome pager response. Patient admission histories. Transition of care involvement. Patient and Caregiver medication counseling. Participate in educational initiatives. Provides oversight for quality assurance of clinical pharmacy programs to improve quality of care. Contribute to professional development of staff, including Pharmacists and Clinical Pharmacists. Identifies and develops policies and procedures that optimize the pharmacy operations while meeting accreditation standards and applicable regulations. Evaluates and reports measures of patient encounter productivity and identifies methods to enhance efficiency of such encounters. Develops and implements clinical practice guidelines that will enhance pharmaceutical care and medication safety. Clinical intervention, adverse drug reaction, and medication error documentation and reporting. Hospital committee involvement, as identified and assigned by the Director of Clinical Pharmacy Services. Identifies and provides expertise for Medication Use Evaluations. Maintains professional competency and certification. Additional Position Specific Responsibilities:Longitudinal responsibilities including on call, staffing and code response. ASHP requirements as outline in residency contract. Minimum Qualifications:To qualify you must have a Doctor of Pharmacy degree from an ACPE accredited College of Pharmacy. Licensure or eligible for licensure in the State of New York. Preferred Licenses: Pharmacy Intern Permit-NYS, Pharmacist License-NYS - Qualified candidates must be able to effectively communicate with all levels of the organization.
