Completes successfully the IV sterile technique module including workbook, media fill challenge, and calculations test and maintains competency requirement. Demonstrates knowledge of basic IV technique, IV hood/glove box and understands the USP 797 requirement in IV preparation. Demonstrates the ability to aseptically prepare IVPB, syringes, large volume IV, ADD-Vantage and Add-A-Vial preparations. Demonstrates knowledge and ensures compliance with the laws/rules/regulations governing the labeling of IV vials, syringes, compounded IV products including concentration, beyond-use-dating, storage requirements and initials. Demonstrates competency in using the PA - Rx to fill pyxis stations. Picks accurately medications based upon Par. X requirements. Refills accurately medications into pyxis stations using scan technology. Demonstrates the ability to load new medications into pyxis. Enters accurately the date of the next medication to expire EACH TIME the medication is refilled. Fills medication lists accurately. Labels drawers with correct patient name and room number. Delivers accurately all medications to correct patient bin in a timely manner. Completes monthly nursing station inspections as assigned. Removes all outdated medications. Responds to stock outages immediately. Demonstrates the ability to change pyxis paper, fix drawer jams, retrieve reports as necessary. Demonstrates the ability to accept employee prescriptions including ensuring completion of associated paperwork. Enters accurately prescription into the computer system. Fills accurately the prescription. Faxes prescription and paperwork to Fuld Outpatient Pharmacy and places original paperwork into the transport container, delivers container to Security Desk timely. Completes accurately the transport log, loads checked prescriptions into the transport container. Retrieves promptly transport container from Security, unpacks container, places prescriptions in alphabetical containers, refrigerator or narcotic bin as applicable. Dispenses correct prescription to employee utilizing QS 1 signature system and Quick. Charge. Demonstrates the ability to access patient profiles and retrieve ADT information. Demonstrates the ability to prepare WORX jobs as needed i.e., fills, labels (IV/ PB/ SYR) production lists, profiles, etc. Fills accurately medication orders, matching label to doctor's order. Processes STAT medications as a priority. Fills crash cart and anesthesia trays accurately and assigns expiration dates. Follows good manufacturing practices when unit dosing liquids and solids. Complies with expiration date requirements. Delivers controlled substances in accordance with hospital policy. Maintains required paperwork. Reports any irregularities (discrepancies, etc) to a pharmacist in a timely manner. Assists in the ordering process by using the want book as evidenced by documentation in the want book. Demonstrates the ability to order medications from the wholesaler when applicable. Ensures the order is checked-in and put away in a timely manner, especially those requiring refrigeration. Follows the procedure for borrowing and loaning medications with other institutions or retail pharmacies. Assists in reviewing product expiration dates as evidenced by lack of expired medications in the active stock area. Assists in returning medications to stock and credits patients appropriately as evidenced by observation of a pharmacist. Stocks all areas accurately, as evidenced by lack of errors. Accurately and promptly fills floor stock requisitions and stock for off site locations. MINIMUM REQUIREMENTS - Education: High school diploma or equivalency. Experience: Current pharmacy technician certification (C - Ph. T) or acquire C - Ph. T within 12 months of employment. Current NJ Board of Pharmacy Registration or acquire the registration within 6 months of employment. Knowledge and Skills: Basic computer and mathematical skills. Usual Work Day: 8 Hours
