The Supervisor of Discharge Planning Coordinators provides direct leadership and operational oversight for the Discharge Planning Coordinator team, ensuring efficient, high-quality support for patient discharges and care transitions. The Discharge Planning Coordinator role is responsible for managing and coordinating care referrals, arranging post-acute services, and supporting the discharge process to promote safe, timely, and effective patient transitions. This position is responsible for staffing, scheduling, workflow management, performance evaluation, and professional development of the team. The Supervisor works in close collaboration with Case Managers, Social Workers, physicians, nursing staff, and post-acute providers to optimize patient throughput, improve discharge efficiency, and maintain high standards of service and compliance.
High school diploma, certificate of attendance, certificate of completion, GED or equivalent training or experience required.
Associate Degree preferred or Bachelor’s degree in Nursing, Healthcare Administration, Social Work, or related field preferred.
Equivalent combination of education and relevant experience will be considered.
Prior leadership or supervisory experience strongly preferred.
Familiarity with post-acute care services, payer requirements, and regulatory guidelines.
Strong leadership abilities with proven experience managing and developing staff.
Excellent organizational, time management, and multitasking skills.
Effective interpersonal and communication skills, with the ability to collaborate across disciplines.
Proficient in using electronic medical records (EMR) and Microsoft Office Suite.
Demonstrated ability to analyze workflows and implement process improvements.
Strong problem-solving skills with a focus on patient-centered solutions.
Knowledge of Medicare, Medicaid, and commercial insurance discharge requirements.
Minimum Experience Required: 3 Years
