Rutgers, The State University of New Jersey is seeking a Habilitation Counselor for the Camden Residential Program within Rutgers University Behavioral Health Care. The primary purpose of the Habilitation Counselor is to provide day-to-day direct care, face-to-face case management and supportive services to individuals who reside in UBHC Housing programs as directed by the Program Supervisor in accordance with all regulatory requirements. Among the key duties of this position are the following: Assumes direct care responsibilities for individuals served in the Residential program and provides face-to-face supportive counseling, crisis intervention, psycho-education, skills building, and case management services as indicated. Involves individuals served and family/significant others in jointly formulating Residential Treatment/ Individualized Recovery Plans reflecting measurable goals and objectives based on individual’s input, clinical assessment, and treatment team recommendations in accordance with all regulatory requirements. Collaborates effectively with other departments, staff, referral sources, other service providers and social support networks to provide services and rehabilitative care for individuals served. Completes Individualized Residential Treatment/ Recovery Plans one week before due date and obtains all necessary signatures by Plan due date as monitored by documentation tracking. Ensures that all relevant attendance records and logs reflect the individual’s attendance. Collaborates effectively with members of the department and with external departments, staff, referral sources, other service providers and social support networks to provide services and rehabilitative care for consumers individuals and families. Demonstrates positive participation in all department team and staff meetings and with other treatment providers. Equivalent education, experience and/or training may be substituted for the degree requirement. Certifications/ Licenses Applicants possessing a degree in Social Work must have an active New Jersey certification/licensure as required by the State Board of Social Work. Valid driver’s license. Equipment Utilized Phones, Laptop/desktop, scanner, printer, fax machine, motor vehicle Physical Demands and Work Environment PHYSICAL DEMANDS: The employee spends much time driving, lifting, walking, standing, bending and talking with individuals in the community and residential settings located throughout the County. Transport of individuals served. The employee can expect to lift up to 30 pounds. WORK ENVIRONMENT: 24-hour responsibility for residential services for adults diagnosed with severe and persistent mental illness, forensic histories, and drug/alcohol abuse with co-occurring medical issues. Intense work environment with ongoing crisis assessment and intervention. Position demands constant attention to multiple programmatic needs.
