Develops and implements a patient focused nursing plan of care in collaboration with the patient/family. Cares for patients in accordance with established best practices. Documents patient care given. Monitor, record, and communicate patient condition as appropriate to care team, physician, patient, and family. Assist patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation. Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs. Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances. Works as an advocate for the physical and emotional well-being of the patient. Maintains the privacy and security of all confidential and protected health information. Use and disclosure only that information which is necessary to perform the function of the job. Contributes to and assists in maintaining a pleasant patient environment in a safe and orderly manner. Applicants should have a current Michigan RN license (or attain within 90 days of hire) and possess an associate degree from an accredited nursing school (bachelor's degree preferred).
