We have an exciting opportunity to join our team as a Health Educator. In this role, the successful candidate The health educator will facilitate evidence-based sexual healthworkshops for adolescents in a high school and middle school settings, implement risk reduction counseling andprovide sexual health education in school based health center. Schedule will be split between school based health center and Project Reach Youth community center. Job Responsibilities:1)Work with Program Coordinator and school staff to schedule workshops in high schools at the FHC school-based health center and other schools in CB - Os in Brooklyn .. Conduct evidence-based sexual health workshops using Making Proud Choices curriculum..3. Administer sexual health risk assessment surveys to all workshop participants 4. Meet individually with workshop participants identified as high risk to provide risk reduction counseling connect them to the FHC School Based Health Center or other social services .. Promote FHC School Based Health Centers services 6. Provide sexual health education to youth at FHC School Based Health Centers 7. Assist patients in enrolling the Family Planning Benefit Program as needed .. Provide rapid HIV counseling at FHC School Based Health Centers and complete all related documentation .. Participate in project SAFE community events to connect youth STI and HIV testing .0. Participate weekly staff meetings at Project Reach Youth 11. Submit workshop attendance information in ETO - Minimum Qualifications:To qualify you must have a HS Diploma. Prior work experience with community health outreach. Health related background. Preferred Qualifications:At least three years providing services to young adults. Good interpersonal skills. Workshop facilitation experience. Interest in public health/sexual health education. HIV testing experience. Experience with electronic medical record system (EPIC)Second language: Spanish, Haitian Creole, Mandarine, Arabic.
