Serves as liaison and coordinator between patients and social services as needed to assist the client in achieving success in the developed plan. Refers individuals to community agencies based on needs and problems identified in the assessment; coordinates services needed; Assists client in accessing private and public services and community resources such as Department of Human Services, food banks, housing, child care, education, mental health centers, and medical providers, concerning the needs and problems of patients. Follows up on referrals made and actions to be taken by the client; may evaluate and adapt educational materials for use with clients; may act as a mentor and conduct outreach activities. Participates in the development and execution of individual interdisciplinary treatment plans as appropriate by evaluating patients’ strengths, needs and goals and makes treatment recommendations accordingly. Minimum Qualifications Graduation from an accredited college or university with a master's, or doctorate degree in social work and one (1) year of full-time work experience in professional social work or a social work-related field. A valid driver’s license is required. ADDITIONAL REQUIREMENTS:Prior to operating a County-owned vehicle, all employees are required to complete the HC Defensive Driving course and/or an Emergency Vehicle Operations Course (EVOC) provided at the department level. This position is deemed essential during inclement weather situations and must report to or remain at work, even when administrative closings are announced, as determined by the Department Head. PREFERRED QUALIFICATIONS:A Master's of Social Work Certificate (CMSW) or License of Clinical Social Work (LCSW) from the State of Tennessee is preferred. PHYSICAL REQUIREMENTS:Work requires working with high risk, confrontational, mentally, and physically ill, and economically disadvantaged populations with potential exposure to infectious diseases. Work may require making home visits; may involve working after regular business hours in high crime areas; may work at various sites or may be required to make presentations and attend community meetings.
