In 1971, Yale University established Yale Health to provide health services to its facility, staff and students through a multidisciplinary health maintenance organization located on campus. Yale Health has more than 50,000 members including students, staff, faculty and their families who come from every state in the country and almost every country in the world. With over 50 years of service to the Yale community, our state-of-the-art facility at 55 Lock Street, is where our members receive most of their care from our 150 providers. It is a 144,000 square foot medical facility with over 90 exam rooms, an Acute Care Department, a 15-bed inpatient facility with two negative pressure rooms, a diagnostic imaging suite including MR and CT scan, x-ray and ultrasound and a full-service retail pharmacy. Reporting to the Assistant Director of Student Medicine and Vaccine Services, this position will operate as the department Immunization nurse, administering vaccinations and medications needed per standing order or written orders by patient’s Primary Care Provider. Experience and interest in immunizations, all ages, but primarily adult population supporting primary care. Interest in Travel Health and patient education for travelers. This role includes daily management of Epic inbasket and patient messages, along with Outlook email account. Cross training coverage to Student Health/ Athletic Medicine primary care departments, as needed. This role provides direct nursing care that is safe, effective and evidence based. Role includes assessment and triage of walk-in patients. Vaccine workflows also include the Medicare population, as well as Immigration vaccination needs. Care coordination for travel health, consisting of chart preparation, maintenance of clinical schedules, administration of travel vaccinations, and basic travel health clinical knowledge. Fosters ongoing improvements of service and a culture that prioritizes safety. Role requires the ability to interface with key clinical partners, ability to make difficult decisions and facilitate resolution. General knowledge of Health On Track database, workflows for incoming students and employees, and potential verification assistance opportunities. Additional responsibilities include:Supports the Immunization Care Team by assuming responsibility for vaccination and medication administration. Coordinates and/or delivers direct nursing care to patients and collaborates with members of the multidisciplinary team to support the work of the team, department activities, and the accomplishment of positive patient outcomes. Clinical judgment and decision making in accordance with clinical practice guidelines, protocols and clinician orders, including follow up, preventive health, maintenance, teaching and education, including high demand times. Assume responsibility for evaluation and disposition of scheduled patients as well as same day- walk in patients. Delegates responsibilities to appropriate members of the care delivery team based upon an understanding of others' roles, scope of practice, knowledge, and capabilities to ensure optimal utilization of personnel. Assure appropriate documentation of patient encounters, including patient teaching, self-care advice, travel health information, vaccination summaries, and coordination of health services as indicated. Responsible for training and orientation of members on the care team. Identify education needed to assure competent delivery of patient care for oneself and others in the department. Contributes to a positive work environment by being responsive to the need for change. Maintains a collaborative work relationship with staff members through respect and recognition of cultural and human diversity. Successfully completes the competency-based orientation within designated time frame. Identifies self-learning needs and actively participates in meeting them in collaboration with the leadership team. Sets and achieves professional goals. Delivers excellence in patient care through compliance with all patient safety and regulatory initiatives. Uses resources in a fiscally sound manner. Compliance with University, State and federal healthcare regulations. Schedule: Part-time, 20 hours; 2.5 days a week, between 8:30-5:00; Occasional early mornings, evenings, weekends, holidays/recess periods may also be required. Required Skills and Abilities 1. Outstanding communication and interpersonal skills .. Competent clinical triage skills and critical thinking skills .. At least 2 years in a medical/surgical setting, critical care setting, preferably in a hospital or outpatient/ambulatory facility .. Evidence of maintenance of professional skill and competency, attendance at educational programs such as seminars and workshops that are appropriate to the field .. Active membership in professional organizations. CPR certification .. Computer proficiency as it relates to the use of an EMR, Outlook email, and other computerized databases such as Excel, Salesforce, and Microsoft Teams .. Excellent attendance, punctuality, and reliability as attested to by references. Preferred Skills and Abilities. Demonstrated experience in an in-patient hospital unit in relevant specialty. Experience in Immunizations administration as well as administration of medications. Principal Responsibilities 1. Required License(s) or Certification(s) Licensure as a registered nurse in the State of Connecticut. Physical Requirements May require prolonged standing, walking, use of sight, hearing and touch, moving/lifting patients of varying weights.
