What You’ll Do - Provide skilled nursing care to patients in their homes - Perform patient assessments and develop individualized care plans - Educate patients and caregivers on treatments and disease management - Coordinate care with physicians, families, and interdisciplinary teams - Monitor patient progress and communicate changes in condition - Maintain accurate documentation and ensure quality patient outcomes. What You’ll Need - Active, unrestricted RN license in the state applying for - Minimum of 1 year of clinical nursing experience - Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment - Reliable transportation, valid driver’s license, and current auto insurance - Willingness to travel throughout assigned coverage area. Preferred Qualifications - Home health or hospice experience - Experience with HomeCare Home. Base - Associate or Bachelor’s Degree in Nursing preferred.
