Responsible for the overall administration of the HH teams at assigned site. This includes clinical supervision of HH staff, coordination of client services, staff recruitment and hiring, staff training, community and case finding, weekly, monthly and quarterly reporting of site information to the Deputy and Program Director, and implementation of program policies and procedures. Responsible for site, delegation of caseloads, conducting and documenting weekly chart reviews. POSITION SUMMARY:Under the direction of the Program Manager/ Coordinator of PEP and Pr. EP, the Pr. EP Specialist conducts intakes and assessments; coordinates referrals and medical appointments; troubleshoots patient insurance issues; conducts follow up with clients and provides retention and adherence counseling to PEP/ Pr. EP candidates. DUTIES AND RESPONSIBILITIES: Assesses needs of patients seeking and accessing PEP and Pr. EP related services. Provides direct service such as intakes, navigation, education and follow up around PEP and Pr. EP services. Assists with data collection for AIDS Institute Reporting System and CHN's Electronic Medical Record. Coordinates medical services with clinic staff. Provides a wide range of community outreach, education, and risk reduction interventions. Participates in health fairs, tabling and joint health activities with other community agencies. Participate in trainings and support services. Maintains strict confidentiality of all patient related information. Facilitates PEP/ Pr. EP related services for health center patients as appropriate with respect to their confidentiality and privacy. Compliance with Employee Health Services. Performs other related duties, as assigned. EDUCATION AND EXPERIENCE required:Associate degree or at least 2 years' related experience working with patients in a health care setting and experience in HIV/ AIDS. Member of the targeted communities and/or the targeted service groups. KNOWLEDGE, SKILLS AND ABILITIES REQUIRED: Strong knowledge of HIV prevention initiatives. Strong clinical background with theoretical training in human behavior. Practical experience in crisis intervention in an acute care setting. Strong verbal and written communication skills Demonstrated proficiency in Microsoft Word and Excel Respect for the rights of individuals to make their own decisions about their medical care. Ability to communicate easily and display a cordial manner towards individuals from a variety of socio-economic, cultural and religious background. Bilingual Spanish speaking skills desired#LI-DF 1
