Other duties as assigned. Communicates with patients and families after discharge to address concerns and ensure understanding of follow-up care and functional assessment. Collaborates with the multidisciplinary team to ensure high-quality patient care. Participates in community education as needed. Maintains professional relationships with all customers and completes assigned work comprehensively and in a timely manner. Provides telephone and face-to-face consultations with patients, answering questions, providing educational information, and reviewing discharge information.
