Responsible for participating in a collaborative process to monitor and evaluate options and services to meet an individual’s health needs. Through communication and available resources, provides quality, cost-effective care utilizing selective benefits and alternative services best suited for the patient, while ensuring optimal outcomes. Key in ensuring appropriate sequencing of treatment goals and implementation of a comprehensive discharge plan. Demonstrates knowledge and skills necessary to provide case management services. Required Skills:Current, valid, and active license to practice as a Licensed Practical/ Vocational Nurse, Respiratory Therapist, Certified Occupational Therapy Assistant, or a Physical Therapy Assistant in the state of employment required; or a Bachelor’s or Master’s degree in Social Work (BSW or MSW) required. Current BLS certification from a Vibra-approved vendor required. Additional Qualifications/ Skills:Experience in a rehabilitation or acute care setting preferred. Proficient in collecting and retrieving material from medical records. Ability to project a professional image. Knowledge of regulatory standards and compliance requirements. Strong organizational, prioritizing and analytical skills. Ability to make independent decisions when circumstances warrant.
