The Clinical Pharmacy Specialist, Ambulatory Care is responsible for providing comprehensive pharmaceutical care to patients through clinical services, formulary management, development of policies and procedures, and integrated drug distribution. Individual will be responsible for providing recommendations and/or assuming responsibility for disease-related activities, anticoagulation, or other medication regimens as prescribed by provider. Core Responsibilities and Essential Functions:Provide recommendations and/or manage therapy associated with anticoagulation, glycemic management, antimicrobial stewardship, or other medication regimens as prescribed by the provider.a. Document clinical interventions on an ongoing basis.b. Demonstrate a basic understanding of distributive responsibilities including computer functions.c. Assist in investigational drug studies when needed.d. Provide on-call clinical pharmacy services as needed.e. Perform medication management activities as outlined per Wellstar policies. Educational responsibilitiesa. Develop and provide educational programs/inservices for health care practitioners and patients as required.b. Provide drug information when needed or requested.c. Serve as a preceptor/instructor for students from accredited colleges/schools of pharmacy.d. Serve as a preceptor for the Pharmacy Residency Program (where applicable).e. Assist in developing drug protocols. Participate on assigned Committeesa. Participate as a member of the Wellstar Pharmacy and Therapeutics (P&T) Committee, as well as other System and facility-specific committees as assigned or as deemed necessary.b. Identify target medications/therapies, develop criteria, and implement data collection for Medication Use Evaluations (MUE) or other ambulatory care-relatedstudies/initiatives. Tabulate and present completed MUE data to the P&T Committee.c. Monitor metrics regarding medication management activities.d. Assist in formulary maintenance by critiquing new entities, comparing medication classes, and developing therapeutic interchanges where applicable.e. Assist in preparing for and maintaining compliance with regulatory agency (e.g., The Joint Commission) standards/requirements. Performs other duties as assigned. Complies with all Wellstar Health System policies, standards of work, and code of conduct. Required Minimum Education:Bachelors Pharmacy or Doctorate Pharmacy-Preferred. Required Minimum License(s) and Certification(s):RPH - Reg Pharmacist. Additional License(s) and Certification(s):Board Certification (e.g., BCPS, BCACP, BCGP, etc.), strongly preferred Preferred or. Specialty credentialing (i.e., Certified Diabetes Educator) preferred or. Pharmacist Registration in state of Georgia within 180 Days Required. Required Minimum Experience:Minimum 3 years clinical pharmacy experience or Completion of a PGY 1 Pharmacy Residency Required and. Minimum 2 years clinical pharmacy experience in Ambulatory Care or Completion of a PGY 2 Ambulatory Care Residency Required and. Minimum 2 years Experience with anticoagulation, diabetes mellitus, heart failure, COPD Preferred.
