HOPE ~ HEALTH ~ HEALING Seeking a Case Manager to join our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our South Mountain Recovery Health Center. Full-Time Employed: 40 hours/week M-F 8:30 a - 5:30 p Location: 3540 E. Baseline Rd. Unit 150, Phoenix, AZ 85042 Additional Language Differential Pay Available NOW OFFERING AN ADDITIONAL $1.50/hr SMI DIFFERENTIAL Full Benefits Package, including 401 K Generous PTO/ Sick Time (4 weeks in year 1) No experience required with a bachelors degree in behavioral health - we will train you New grads welcome Duties Include: Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals Completes assessments, crisis plans and individualized service plans Works collaboratively with the clinical team to engage, educate, communicate and coordinate care with member, their family, behavioral health, medical and dental providers, community resources and others in ensuring that all services prescribed in the individualized service plan are implemented Conducts outreach and follow-up of services including, but not limited to, crisis interaction, missed appointments, and hospital discharges to ensure adequate resources are available and in place.
