Fairview Range is looking for a Registered Nurse Care Coordinator to join our Oncology Department at our Mesaba Clinic in Hibbing, MN. The RN Care Coordinator identifying and managing patient outcomes across the continuum. A Care Coordinator is a registered nurse who manifests a high level of expertise to assess, plan and facilitate treatment for actual or potential complex health problems of individuals, families or communities. The Oncology Care Coordinator coordinates and manages the care of the Oncology patients in order to meet multiple service needs, ensure continuity of services, decrease fragmentation of care in all settings, enhance the patient's quality of life, and ensure cost containment. The care coordinator assists the patient/family in coping with illness, optimizes the patient's self-care abilities, and supports patient's rights to make choices. In addition to managing patients through an episode of illness, the care coordinator focuses on wellness and disease management. The Oncology Care Coordinator has knowledge and skill to provide IV/ VAD Care as well as administer medications through all routes in an outpatient setting. Schedule: This position is full time, working 72-80 hours per two weeks. 8 - hour day/evening shifts and weekends as needed. Internal posting through: 6/1/26 About this facility. Specialty departments include surgical services, orthopedics, OB/ GYN, ENT, allergy, wound care, podiatry, and more. RN Care Coordinator Job Responsibilities. Timely and accurate assessment of the patient's clinical, social, functional, and continuing of care needs through direct observation, data analysis and interviews in order to evaluate the need for intervention. Plans for and identifies immediate, short term, and long-term care needs for patient/client. Sets appropriate patient/client goals and time frames for achievement in order to achieve desired outcomes. Evaluates and measures patient/client response to planning, services, and interventions in order to assure quality of case management services and to make improvements where necessary. Demonstrates ability to communicate appropriate information across service lines to ensure effective and efficient utilization of services. RN Care Coordinator Job Qualifications. Required Qualifications. Associate Degree in Nursing. Current Minnesota RN Licensure. BLS - Chemotherapy/ Biotherapy provider card or willingness to obtain within 6 months of hireOne year of acute, outpatient, and/or ambulatory direct patient care RN experience or one year of recent oncology experience. Preferred Qualifications. Bachelor of Science in Nursing. Oncology specialty certification - Oncology Certified Nurse. Previous case coordination experienceOne-year clinical experience in the Oncology care setting.
