Maintenance and review of patient medication profiles; reviewing and verifying physician medication orders and patient medical record; and checking of IV products. Interacting with healthcare professionals and providers to help promote safe medication use, prevent inappropriate or misuse of medications; provide clinical pharmacy services (e.g., pharmacokinetic dosing, therapeutic interchange, IV/ PO conversions, clinical protocols per pharmacy and pharmacist interventions). Hourly Salary Range: $91.06-$106.67 Bargaining Unit: HX-UPTE Job Qualifications We are seeking an exceptionally-skilled and compassionate pharmacist with the following qualifications: Doctor of Pharmacy Degree. California Pharmacy license ASHP-accredited PGY 2 oncology residency or at least a minimum of 1 year of pharmacy practice experience in oncology. Recent clinical oncology experience (within 5 years)Board Certification in Oncology (BCOP) preferred. Previous infusion pharmacy experience preferred. EPIC/ Beacon experience preferred. Exceptional communication, interpersonal, organizational and follow-through skills. Knowledge of and ability to perform age specific and disease specific pharmacokinetic calculations.
