Essential Duties. As a part of the Sunrise team, supporting our Mission, Principles of Service and Core Values is a fundamental part of this job. The unique responsibilities for this role include but are not limited to the essential functions listed as follows:Concierge Skilled Nursing Duties. Oversees the Skilled Nursing Center entry area for visitors, families and guests/residents, extends a warm welcome and greeting. Serves as the telephone receptionist for the Skilled Nursing Center while meeting and/or exceeding hospitality and quality service standards. Takes complete messages with pertinent information (name, number, message) as appropriate and communicates messages to the intended recipient. Assists guests/residents, families, visitors, physicians and Skilled Nursing team members as requested with information, supplies, records and messages. Performs clerical duties for the Skilled Nursing Center. Documents monthly physician orders on guest's/resident's chart. Maintains nurses stations including forms, files, drawers and cabinets. Sends get well cards to residents in the hospital. Orders Memory Flower for any residents that have passed away. Sends sympathy cards to families and/or responsible parties as needed. Record Keeping. Maintains complete and accurate medical records in compliance with Sunrise Senior Living, Federal, State/ Provincial, Local regulations and regulatory agencies and the standards of excellence promoted by the American Medical Records Association. Ensures proper admissions documentation per admission procedures. Assembles and completes all new charts with guest/resident assigned medical records number, name, physician's name and/or other information pertinent to the guest/resident. Maintains guest/resident charts according to Records Procedure Manual to meet regulations and to ensure ease of handling. Establishes and maintains a chart alphabetical filing system for both current and discharged guests/residents in the medical records office. Manages discharged guest's/resident's charts as outlined in Medical Records Procedures Manual; monitors for incomplete records or notes; notifies the Skilled Nursing Administrator or any deficiencies. Completes all necessary information in the Death Registry when a resident expires. Provides Administration and Nursing leaders with statistical compilation of data as required. Risk Management and Safety. Partners with community team to ensure community is in compliance with national/provincial regulations pertaining to occupational health and safety requirements and promotion of Risk Management programs and policies; adherence to safety rules and regulations. Practices safety procedures at all times including Personal Protective Equip (PPE), fire extinguishers, Safety Data Sheets (SDS), Lockout Tagout procedures. Identifies potential hazards and maintains a safe environment for guests/residents and fellow team members. Adheres to universal care precautions and all other infection control guidelines. Demonstrates knowledge of emergency policy and procedures. Guest/ Resident, Family and Visitor Relations. Responds to guest/resident, family, visitor's inquiries, concerns in a timely and professional manner using the Problem Resolution process. Communicates and assists other skilled nursing team members to accomplish guest/resident care plans. Maintains and protects the confidentiality of guest/resident information. Training, and Contributing to Team Success. Participates actively as a member of a team and committed to working toward team goals. Demonstrates in daily interactions with others, our Team Member Credo. Commits to serving our guests/residents and visitors through our Principles of Service. Contributes in the overall engagement programs and processes (customer and team member engagement) including participating in the team member engagement survey and engagement improvement planning workshops. Attends regular meetings; Town Hall, Department Team Meetings, Cross Over, and others as directed by the Supervisor / Department Coordinator. Participates in annual in-service as set forth by federal and state/provincial regulations. Maintain compliance in assigned required training and all training required by state/province or other regulating authorities as applicable to this role to ensure that Sunrise standards are always met. High School degree / GED required. Proven customer service experience and skills preferably in a skilled nursing / long term care environment. Experience in medical records system preferred. Ability to handle multiple priorities. Possess written and verbal skills for effective communication. Demonstrates good organizational, time management skills and follow through skills. Possess accurate record keeping skills. Demonstrates good judgment, problem solving and decision making skills. Demonstration of proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel) and Sunrise applications with the ability to learn new applications. As applicable, all Sunrise team members who drive a Sunrise vehicle must sign the Driver job description and understand the key essential duties for safety and regulatory compliance
