We are seeking a Clinical Nurse II in our Duke Signature Care clinic located in Durham, NC to be an integral member of the healthcare team and provide direct and indirect care to patients. Job Duties and Responsibilities: Participate in daily huddles with providers and clinical team to coordinate patient care needs. Plan, provide and document professional nursing care in ambulatory settings, utilizing the nursing process, in accordance with physician orders and established policies and procedures, under the supervision of the assistant nurse manager, nurse manager and Health Center Administrator. Perform patient assessment on intake and documentation utilizing double identifiers including reason for visit, vital signs, medication reconciliation, falls risk, goals, learning assessment, PHQ 2/9, SDOH, patient screening for immunizations, and appropriate forms. Participates in own professional development by maintaining required competencies, assessing learning needs, and continuing education as required by the NCBON/ other Certification Agencies. Maintains current knowledge of policies/procedures set forth by DUHS/ DHIP, other regulatory agencies, and those that are clinic specific. (Infection Control, Environment of Care, Joint Commission/ PCMH, etc.) Assist/chaperone provider with procedures (pap smear, breast/rectal exams, shave biopsy, intra-articular injection/aspiration, etc.) Collect and analyze patient data, make recommendations for nursing care plans, evaluate nursing care provided, document and communicate appropriately nursing actions taken and patient responses. Address staff messages, patient prescription refill requests patient calls, prior authorizations, faxing, etc. during work shift; walk-in and telephone patient triage when needed. Perform EKG’s ear lavage, administer medications (PO, IM, ID, SQ, topical, inhalation), immunizations, nebulizer treatments, suture removal, wound dressings and pulse oximetry as needed. PIV insertion for patient hydration and urinary catheterization as ordered by provider. Perform checks, maintains clinic logs for equipment, supplies, medication, refrigerators, and runs controls in accordance with appropriate policy. Clean exam rooms, stock, check for expiration dates, and document on appropriate log. Participate in the identification of clinical, operational and safety related performance improvement opportunities. Monitors inventory and reports to assistant nurse manager or nurse manager as appropriate. Maintains orderly work environment by keeping work station clean/tidy, hallways and patient care areas hazard free. Educates the patient, family, or support person regarding medications, treatment plan, plan of care and documents within the Electronic Health Record (EHR). Monitor and initiate corrective action to maintain the environment of care, including equipment and material resources. Participates in clinic and entity level Performance Improvement activities. Perform phlebotomy, point of care testing, and lab processing in accordance to policy. Delegate task and supervise activities of unlicensed assistive personnel; assist other nursing personnel in the delivery of patient care; Support the development of other staff and formal learners. Coordinate patient movement throughout clinic to ensure consultations, procedures, and other forms of care are achieved in a timely manner in accordance with physician orders. Perform other related duties incidental to the work described herein. Job Eligibility Requirements: Work requires graduation from an accredited BSN or Associate's Degree in Nursing or Nursing Diploma program. All registered nurses without a Bachelor's degree in Nursing (or higher) will be required to enroll in an appropriate BSN program within two years of their start date and to complete the program within five years of their start date. 2 years of clinical experience is required. Adult Medical/ Surgical, ED, Critical Care, Cardiac or previous Ambulatory Care experience with the last 5 years preferred. Must have current or compact RN licensure in the state of North Carolina. BLS required. Job Schedule: Monday-Friday day shift schedule with no weekends or Duke designated holidays.
