Fairview is seeking a Physician to join our Acute and Diagnostic Services (ADS) team. The ADS physician provides high-quality, patient-centered care to stable patients who require diagnostic or treatment services that are more complex than those typically offered in primary care or urgent care, but do not require emergency department stabilization. This role is ideal for physicians who enjoy managing acute and subacute conditions, leveraging advanced diagnostic capabilities, and collaborating closely with primary care providers, specialists, and care teams to ensure seamless transitions of care. Acute and Diagnostic Services locations include: Burnsville, Maple Grove, Edina, Maplewood and Wyoming. Practice Details: FTE: 0.8 ADS Clinic is open Monday through Fridaytypicall hours are Monday - Thursday 9 a-6 p/7 p and Firday 9 a-5 p/6 p. Assess patients, formulate diagnoses, and implement evidence-based care plans Manage acute and subacute medical conditions with a focus on: Exceptional patient experience Value-based care Reducing unnecessary utilization of PCP, urgent care, and emergency department resources Communicate clearly with patients and families regarding diagnoses, treatment plans, and follow-up Provide patient education and encourage active participation in care and recovery Perform medical procedures within credentialed scope of practice Utilize ADS workflows, provider tools, and Fairview referral networks effectively Document care accurately and timely to support quality outcomes and appropriate billing Collaborate with multidisciplinary teams to support care coordination and transitions Respond appropriately to emergent situations Participate in clinical quality improvement initiatives and access planning Support clinic operations, regulatory readiness (JCAHO), and practice management efforts Serve as a clinical resource and leader for colleagues and support staff Participate in provider meetings, committees, and community or professional activities Support education and training of students and clinic staff as requested Qualifications: Graduate of an accredited ACGME/ AOA Internal Medicine or Family Medicine residency training program. BC/ BE with the ABMS or AOA. Licensed or ability to obtain licensure through the Minnesota Board of Medical Practice. Ability to obtain and /or maintain DEA certification for the State of Minnesota.
