Serves as the point of contact, advocate, and informational resource for patients, care team, family/caregiver(s). - Promotes timely access to appropriate care such as helping to schedule appointments - Gathers records, organizes and supports Tumor Boards and Multidisciplinary Clinic(s) and preparation for effective oncology appointments - Monitors and promotes adherence to a care plan, developed in coordination with the patient, PCP, oncologist and family/caregiver(s) - Supports communication between the care team and the patient, facilitates screening and addresses barriers to care and documents all interactions in the patient’s electronic record - Connects patients to relevant community resources, with goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care costs - Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson. OTHER FUNCTIONS AND COMPETENCIES: - Identification and support of high risk patients - Facilitate and attend meetings between patient, family/caregiver(s), care team, andshares community resources as needed - Responsible for regular outreach to patients being served by the Oncology Navigation. Team to check-in, ensure their needs are being met and raise awareness of new orestablished SKCC offerings - Responsible for coordinating services related to patient education, treatment (includingtests and labs), support and follow up care - Support program growth and patient experience in all patient interactions. INTERNAL/ EXTERNAL CONTACTS:Supportive medicine, Social Work, Pallative Care, Community Grants, and Case Management. EDUCATIONAL/ TRAINING REQUIREMENTS:High school or GED required. Bachelor Degree preferred. CERTIFICATES, LICENSES, AND REGISTRATION:EXPERIENCE REQUIREMENTS:Two years experience in clinical or community resource settings; care coordination experience is desirable. ADDITIONAL INFORMATION:Must have experience in a healthcare clinical or community setting, experience in an outpatient oncology setting is desirable. Must demonstrate a service-oriented approach and be a team player willing to support any SKCC patient. Required completion of the George Washington University Cancer Center Oncology Patient Navigator Training, a 20 hour self-paced course for non-clinical navigators, and exhibit competency in each of the 8 domains.
