(not all inclusive)Under the supervision of the Manager, Transportation, is responsible for the direct transportation of physically limited passengers to and from their destinations. Position required constant interaction with caregivers and community agencies while conducting business with Hope PACE. Transport physically limited passengers to varied destinations both timely, safely and door to door. Ensure that the van is in proper working order. Report and document maintenance performed. Assist passengers on and off the van in a safe and courteous manner. Deal with medical equipment and other emergencies that may arise. Follow established scheduled, adjust schedules to meet unanticipated changes , determine the safest and most efficient pick up and delivery routes. Participate as a member of interdisciplinary team. Take part in participant care planning and contribute to notations in medical record. Report important information to proper staff of Hope PACE daily regarding any changes, progress, or unusual events concerning any participant. Participate in the maintenance of a safe, clean and healthful day center environment. May assist with patient care in the day health center when not transporting participants. Represent the company in a positive and professional manner at all times with others, e.g., co-workers, management, vendors, and customers. Consistently represents Hope with integrity and professionalism. Complete all job responsibilities in a thorough and complete manner, utilizing good judgment in all aspects of the job. Carry out duties and responsibilities in a manner that exemplifies excellent customer service. When unsure of a directed task or any part of job responsibilities, proactively seek the assistance of immediate supervisor. Assist with special projects, as requested. All other job duties as may be assigned. Compensation Pay Range: $16.48 - $23.69
