JOB SUMMARY This position works with limited oversight to efficiently provide nursing triage of all patient messages received via telephone or My. Chart. The triage nurse will demonstrate sound clinical judgment that utilizes careful listening, critical thinking and assessment skills. During downtime, the position works with limited oversight to efficiently process patients to appropriate units based on physicians' order whether coming via ED, DA, Transfer, Clinic, Surgery or Procedural Area. EXPERIENCE AND EDUCATION Required. Licenses and Certifications(RN) REGISTERED NURSE Licensure as Registered Nurse by Board of Nurse Examiners for State of Texas Preferred. Experience 2 years of ambulatory clinical nursing experience or emergency room experience JOB DUTIES Utilizes strong listening skill to be able to interpret subtle indications of the patient's complaint so that they can form a thorough assessment of the patient's problem without the ability to visually observe or assess. Follows clinic specific protocols and/or professional judgment and assessment skills to identify the nature and urgency of patient needs to determine the appropriate disposition. Provides patient education and advice to improve continuity of care, avoid unnecessary visits to the emergency room and reduce risks from medical complications. Collects sufficient data related to the presenting problem and medical history and recognizes and mates symptom patterns to those in clinic protocols to be able to provide safe, timely disposition of health-related problems. Refills medications per clinic specific protocols. Establish and maintain ongoing communication with internal stakeholders and external customers while securing the patient's admission or internal transfer. Apply clinical expertise and the nursing process to triage and prioritize admission acuity from multiple locations: direct admits, external transfers, ED, onsite clinics and private physician offices. Ensure smooth functioning of the patient placement process to reduce wait times and avoid unnecessary movement of patients, serves as an expert clinical resource for patient placement Facilitate the direct admit process by interacting with physicians to ensure telephone triage results in appropriate patient placement. Notifies Administrative Coordinator or Manager concerning admissions that do not meet the hospitals criteria. Continually seeks new ways to improve processes and increase efficiencies and takes the initiative to communicate recommendations to Administrative Coordinator or Manager. Duties performed may include one or more of the following core functions: (a) Directly interacting with or caring for patients; (b) Directly interacting with or caring for human-subjects research participants; (c) Regularly maintaining, modifying, releasing or similarly affecting patient records (including patient financial records); or (d) Regularly maintaining, modifying, releasing or similarly affecting human-subjects research records.
