Retrieve all discharged records from nursing units. Verify all discharged records are collected. Researches any records that are not collected. Prep discharged records for scanning into the electronic health record. Scan documents as needed into the electronic health record, ensuring correct patient folder and encounter. Maintains productivity sheet on all manual processes. Assists the Manager of Health Information and Coordinator of Health Information to ensure any new forms have been approved Ensures paper records greater than 90 days post discharged are placed in appropriate containers for destruction. Verifies history and physicals and operative notes are completed for each surgical patient using the daily operating room schedule. Assigns financial information numbers (FIN) to loose documents such as insurance verifications/denials, patient rights, other administrative forms. Other Functions:Maintains strict confidentiality of all information including financial/operational, employee/human resource, healthcare/patient data and information. Works in close collaboration with QCI Technician, Physician Liaison and Analysts Health Information regarding chart retrieval. Performs other duties as assigned. Supervisory Responsibilities:This is not a supervisory role. Travel Required. Travel is expected to be between 10% - 20% of the time. Work Environment:The noise level in the work environment is usually moderate. The setting for this role will include office and healthcare settings. Exposure to controlled temperature environments, both hot and cold, is to be expected. Workplace Exposure:The normal work routine involves no exposure to blood, body fluids or tissues. Persons who perform these duties are not called upon as part of their employment to perform or assist in emergency medical care or first aid or to be potentially exposed in some other way. Physical Demands:While performing the duties of this job, the employee is regularly required to communicate with others. The employee is frequently required to stand, walk, sit, demonstrate hand and finger dexterity in the course of their work, handle or feel, and reach with hands and arms. The employee is occasionally required to climb, balance, stoop, kneel, crouch or crawl. Reasonable accommodation may be made to enable individuals with disabilities to perform the essential duties and responsibilities. The expected physical workload is expected to be Light Work: Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg controls requires exertion of forces greater than that for Sedentary Work and the worker sits most of the time, the job is rated for Light Work. This role is expected to use the following machines, tools, and equipment:Computers. Printers. Scanners. Telephones. Facsimile/copier. Required Qualifications:High school diploma/ GED - General office equipment experience. Knowledge of basic clerical duties and skills: typing, pulling records, filing, photocopying, answering telephones, faxing. Knowledge of release of confidential health information, terminal digit filing and basic office practices. Excellent communication skills. Preferred Qualifications:Previous experience in hospital medical records department Salary Range: $16.22 - $23.52 / Hour
