As a Home Health Speech Language Pathologist, you’ll provide patient-centered care that improves communication, swallowing, and overall quality of life. Evaluate and treat patients using evidence-based speech therapy practices. Develop and adjust individualized care plans based on patient progress. Educate patients and families to support independence and long-term outcomes. Collaborate with nurses, therapists, and physicians to coordinate care. Complete accurate, timely documentation to support clinical and regulatory requirements. Participate in care planning, case conferences, and quality improvement initiatives. Identify changes in patient condition and adjust treatment accordingly. What We’re Looking For. Master’s Degree in Speech-Language Pathology. Active Illinois SLP license (or eligible)Completion of Clinical Fellowship Year (CFY)Minimum 1 year of experience (or strong clinical internship experience)Strong communication, documentation, and time management skills. Ability to work independently in a home-based setting. Valid driver’s license and reliable transportation. Certifications.
