How you’ll make an impact in this role Lead patient care journeys by implementing individualized care plans that prioritize recovery and clinical excellence. Coordinate multidisciplinary teams as the central point of contact to ensure seamless, holistic patient support. Advocate for patient safety by monitoring health status and communicating critical changes for immediate, proactive intervention. Empower patients for life after discharge through collaborative transition planning that ensures long-term wellness. What minimum requirements you’ll need Licensure / Certification / Registration:BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Licensed Registered Nurse credentialed from the Texas Board of Nursing required. Education:Diploma from an accredited school/college of nursing required OR a bachelors level degree not in nursing, an active RN License, and enrolled in an Alternate Entry Masters of Science in Nursing Program with a minimum of 1 year in the program completed.
