Provide physical therapy services under the supervision of a Licensed Physical Therapist - Participate in the coordination of patient care - Implement treatment plans and document patient progress - Educate patients and caregivers on exercises, safety, and mobility - Communicate effectively with interdisciplinary team members - Maintain accurate and timely clinical documentation - Adhere to infection control and safety standards. Qualifications - Graduate of an accredited Physical Therapy Assistant program with an associate degree - Current PTA license or PT Licensure Compact (PTLC) license in the state applying, as applicable - Current CPR certification - Valid driver’s license, reliable transportation, and current auto liability insurance - 1 year experience as a Physical Therapy Assistant - Home health experience - Homecare Homebase (HCHB) EMR knowledge - Strong interpersonal and communication skills. Physical Demands & Work Environment - Ability to lift, bend, push, pull, and perform prolonged standing and walking - Manual dexterity and hand-eye coordination required - Ability to work under moderate stress to meet deadlines and patient needs - Work performed in patient homes under varying conditions.
