We have an exciting opportunity to join our team as a Pharmacist. In this role, the successful candidate is responsible for all aspects of medication therapy, including review of all new medication orders and reconciliation of home medications. Job Responsibilities:Maintain active NYS Pharmacist license in good standing. Responsible for triaging work appropriately to minimize order turn-around time and treatment delays. Perform medication reconciliation at point of order verification. Accurately verifies and dispenses medication orders, reviewing medications prepared for dispensing for accuracy and escalating and correcting any errors. Verifies Computerized Provider Order Entry (CPOE) orders and makes necessary corrections in accordance with hospital policy and procedure and NYS and federal regulations. Enters medication orders accurately from written orders and discontinues medications as applicable per hospital policy. Use Electronic Health Record (EHR) tools and Clinical Decision Support (CDS) to screen for potential errors that may lead to adverse drug events and escalate appropriately. Communicates to the appropriate medical, nursing and pharmacy teams regarding issues affecting patient care including but not limited to medication and dose verification, drug interactions, potential AD - Es etc. Documents interventions, notes, and allergies properly in EHR using appropriate applications. Maintains accurate patient profiles including but not limited to appropriate updates to allergy information and other patient factors. Responsible for ensuring that controlled substances are handled in accordance with federal, state and local regulations, and hospital policies and procedures. Ensures compliance with policies and procedures related to working in the sterile and non-sterile preparation areas and special handling requirements of sterile biohazardous products in accordance with USP chapters 795/797/800. Maintains proper records and logs to satisfy regulatory requirements including temperature monitoring, sterile and non-sterile compounding, team member competencies etc. Assists with staff training and development as assigned. Inventory management including but not limited to placing orders with designated distributor, managing Automated Dispensing Cabinet (ADC) and Anesthesia Workstation (AWS) par levels and inventory, managing supply inventory. Other duties as assigned. Minimum Qualifications:To qualify you must have a Bachelors degree in pharmacy Current New York State Pharmacy License Excellent communication (both oral and written), organizational, detail oriented, problem solving, and record keeping skills Basic computer skills Familiarity of USP 797 and 800 guidelines 1-3 years Pharmacist Experience. Required Licenses: Pharmacist License-NYS - Preferred Qualifications:Post Doctoral PharmD. Experience in an OR/ Procedure area setting;ADC management experience .-3 years Hospital Experience.
